You’ll need to provide:

• Child’s name, date of birth, and health number
• Parents’ names
• Family’s address, phone, email
• Contact information of physicians or health care professionals involved in child’s care

Physicians:

For urgent or end-of-life referrals or inquiries, please page the Palliative Care Physician on call at 604-875-2161.​​
